The Problem With the “Just Take TRT” Mentality

Men’s clinics are everywhere today echoing the same sales pitch: “Are you tired, moody, losing muscle? It’s just low testosterone—let’s get you on TRT.”

But here’s the problem: andropause is not simply a testosterone deficiency—it’s a system failure. Declining testosterone isn’t random bad luck or just aging. It’s the body’s way of signaling that toxicity and metabolic dysfunction are interfering with healthy hormone production.


What Andropause Really Looks Like

Men who present with “low-T” often report a wide range of symptoms, including:

• Low libido and lack of morning erections

• Low muscle mass and decreased stamina

• Fatigue and slow recovery from workouts

• Increased body fat

• Memory lapses and brain fog

• Lack of motivation and feeling down


Clinically, andropause is never just about testosterone. These symptoms point to a broader imbalance—often involving insulin, estrogen, and metabolic health.


How Insulin Fuels Andropause

Too much insulin plays a major role in accelerating age-related testosterone decline. Think of it like a thermostat: if the heater is blasting, the thermostat tells the furnace to stop. High insulin sends the same message to testosterone production.


Here’s how it works:

• Insulin resistance reduces sex hormone–binding globulin (SHBG), which lowers circulating testosterone. Normally, this would increase free, unbound testosterone—but that assumes total testosterone is normal. The problem is that insulin resistance also lowers total testosterone, so free testosterone drops as well. With low SHBG, its ability to buffer hormone fluctuations and prolong testosterone’s half-life is impaired, causing testosterone to be metabolized more quickly.

• Visceral fat storage increases, and fat tissue converts testosterone into estrogen via the aromatase enzyme.

• Inflammation and oxidative stress from excess insulin damage Leydig cells in the testes, further reducing testosterone output.

The result? A vicious cycle where rising insulin leads to higher estrogen, lower testosterone, and worsening metabolic health.


The Estrogen–Testosterone Imbalance

Excess estrogen worsens andropause by disrupting the hypothalamic–pituitary–gonadal axis. When estrogen climbs too high relative to testosterone, it suppresses luteinizing hormone (LH)—the key signal that tells the testes to make testosterone.

At the same time, elevated estrogen increases SHBG from the liver, which lowers free, active testosterone, making it unavailable for tissues. This imbalance leaves men with classic andropause symptoms: fatigue, mood changes, muscle loss, low libido, and weight gain.


Resetting Hormones Through Metabolic Balance

The solution isn’t just more testosterone injections—it’s addressing the root causes.


Fixing Insulin Resistance

• Keto as a short-term intervention (8–12 weeks): lowers insulin, reduces visceral fat, and resets metabolism.

• Transition to a whole-food, low-carb Mediterranean diet: adds flexibility while keeping carbs controlled.

• Intermittent fasting (2–3 days per week): helps reset insulin sensitivity without overstressing the body.


Supporting Estrogen Clearance

• Liver support nutrients: choline, vitamin C, vitamin E, glutathione, and sulfur-rich cruciferous vegetables.

• Gut support: short-chain fatty acids (SCFAs) to prevent estrogen reabsorption.

• Reduce exposures: identify and minimize xenoestrogens (from plastics,

pesticides, chemicals, etc.).


Why TRT Alone Misses the Point

Slapping testosterone onto a broken system doesn’t restore health. Sure, TRT may boost your lab numbers and give you a short-term “high,” but if your liver is sluggish, your gut inflamed, or your body overloaded with toxins, you’re just throwing fuel on the fire.

The truth is, andropause is neither a quick fix nor something to dismiss as “just getting old.” Restoring balance, removing interferences, and supporting metabolic health is what allows testosterone to normalize naturally—and sustainably.
